Written analysis of an incident: timeline, contributing factors, what worked, what didn't, action items. Blameless ones focus on systems, not individuals; useful ones produce committed action items that actually ship. Not the same as RCA — incidents rarely have a single 'root cause,' more a contributing factor cocktail.
